This check list will help to insure that you have met all of the course prerequisites,
and that you have all of the documentation to show it. If you are missing any of the
documentation, your enrollment, and/or final certification may be delayed or denied.
OCEAN DIVERS IDC ENROLLMENT FORM
A signed copy of this form is required for each IDC candidate.
Be sure that you fill out the form completely.
PADI IDC/IOC APPLICATION
A signed copy of this form is required for each IDC candidate.
Be sure that you fill out the form completely.
PADI MEDICAL STATEMENT
A signed copy of this form is required for each student, for any PADI SCUBA training course.
If you have any of the pre-existing conditions, you will need to get the approval, and signature,of your physician, to engage in SCUBA diving/training. Since some doctors are not familiar withthe special needs/risks of SCUBA diving, the form includes basic guidelines and other informationfor the examining physician.
